Mania

1. ELEVATED: Have you ever felt more upbeat, driven, or energized than usual, even if it was an uncomfortable, wired, or anxious energy?

2. IRRITABLE: Have you ever been angry, irritable, or quick to argue for several days?

Do you want to continue with the full mania questions (eg, if patient has recurrent depression or is high-risk for bipolar)?


On the days when you felt more up or irritable [USE THEIR WORDS], did you also...

3. CONFIDENCE ...Feel more confident, like you can achieve great things? Or more demanding, pushy, or self assured?

4. DECREASED SLEEP ...Sleep less than usual but still kept going the next day, even if you wished you could get more sleep?

5. TALKATIVE ...Talk more than usual? Or did people think you were loud or hard to interrupt?

6. RACING THOUGHTS ...Have racing thoughts, multiple trains of thought, or thoughts that moved too fast to keep up with them?

7. DISTRACTED... Get easily distracted by small things or by your own thoughts? Or jump quickly from one activity to another?

8. HYPERACTIVE... Do a lot more than usual, like taking on extra projects or being more socially, sexually, or physically active? Or did you feel more driven or restless even if you didn’t know what to do with the drive?

9. IMPULSIVE ...Jump into things without considering the consequences? Or do things that others thought were extreme, impulsive, or out of character for you?

Are you usually this way [USE THEIR WORDS], or is this marked change from your usual self?

DURATION: Were you ever more or less this way [USE THEIR WORDS] for at least 4 days in a row? For at least 7 days in a row?

What was the biggest problem these symptoms caused? What is the most extreme thing you did? Did it cause major legal, financial, job, school, or relationship problems?

Have you ever gone to the hospital, emergency room, or partial-hospital program because of these symptoms?
